Foam Grips
The foam grips that come with the bar can become very uncomfortable, and after a few pull-ups they make it very difficult to maintain a tight grip. So I recommend using some of these.
Height

I've used this product systematically since Feburary 2012 in step with my amazon account, and i have not had any major problems (i'll keep this updated if it breaks or anything).
